Can I have certain lesson plan components in my layout?

How can I created a lesson plan layout that includes all the components that I want.  I would like standards, objectives, lesson activities/8 step, lesson steps, instructional approach, assessment, guided reading focus, guided writing focus, blooms taxonomy, muliple intelligences, wida, webbs depth of knowledged and 21st century skills.
